Achilles Tendon Repair

Surgical treatment for ruptured Achilles tendon to restore strength and function.

Overview

Achilles tendon repair is a surgical procedure performed to reconnect a partially or completely ruptured Achilles tendon. The Achilles tendon connects the calf muscles to the heel bone (calcaneus) and is essential for walking, running, jumping, and pushing off the foot. When the tendon ruptures, the torn ends may separate, disrupting normal ankle function and strength. Surgical repair restores tendon continuity by bringing the torn ends together and securing them with specialized sutures.

Although not every Achilles tendon rupture requires surgery, operative repair may be recommended for active individuals, athletes, and patients with significant tendon separation. Surgical treatment can reduce the risk of re-rupture compared with nonsurgical management, although both approaches can produce favorable functional outcomes.

When is Achilles Tendon Surgery Recommended?

The decision to perform Achilles tendon repair depends on the severity of the injury, degree of tendon separation, functional demands, and overall health. Surgery may be considered in patients with:

  • Complete Achilles Tendon Ruptures: Full-thickness tears resulting in loss of tendon continuity.

  • Significant Tendon Separation: Ruptures where the torn tendon ends may not adequately approximate with immobilization.

  • High Physical Demands: Athletes and active individuals who rely on powerful push-off movements during running, jumping, or sports.

  • Chronic or Delayed Ruptures: Injuries that have developed tendon retraction, scarring, or persistent functional weakness.

  • Failed Non-surgical Treatment: Persistent dysfunction or re-rupture following conservative management.

Achilles Tendon SpeedBridge Procedure

Achilles tendon repair is typically performed as an outpatient surgical procedure under general anesthesia. Used on professional athletes, the Achilles SpeedBridge procedure is a cutting-edge surgical approach to a torn Achilles tendon. Using a smaller incision, the tendon is reapproximated directly into the heel bone, restoring the native attachment of the Achilles tendon.

Large ruptures or older, chronic ruptures with significant tendon degeneration may not be a candidate for the Achilles SpeedBridge repair. Therefore, it is important that we obtain an MRI and perform a full evaluation to determine if this procedure is best suited for your recovery.

Recovery Timeline

Recovery following Achilles tendon repair requires a structured rehabilitation program to protect the healing tendon while gradually restoring ankle motion and strength.

  • Weeks 0–2: Initial immobilization in a splint or cast, typically with the ankle positioned in plantarflexion. Weight-bearing restrictions depend on the surgical protocol.

  • Weeks 2–6: Transition to a controlled walking boot with heel wedges. Protected weight-bearing and gentle rehabilitation may begin as directed.

  • Weeks 6–12: Gradual progression of ankle mobility, weight-bearing, and strengthening exercises under physical therapy.

  • Months 3–6: Continued strengthening, balance training, and progressive return to lower-impact activities.

  • Months 6–12: Gradual return to running, jumping, and athletic activities when adequate strength and functional milestones have been achieved.

Full recovery may take 12 months or longer. Rehabilitation timelines vary based on the surgical technique used and personalized goals.
