Toenail fungus, medically known as onychomycosis, is a common fungal infection that affects the toenail and surrounding nail structures. It typically develops when fungi enter through small openings in the nail or surrounding skin, leading to gradual changes in nail color, thickness, and texture.

Fungal toenail infections may cause nails to become yellow, white, brittle, thickened, or crumbly. The infection often begins near the tip or side of the nail and may spread deeper over time. Risk factors include aging, excessive moisture, athlete's foot, nail injuries, tight footwear, and certain medical conditions such as diabetes. Because nail trauma, psoriasis, and other nail disorders can resemble toenail fungus, proper diagnosis is important before beginning treatment.
Signs & Symptoms
Toenail fungus often develops gradually and may affect one or multiple toenails. Common symptoms include:

We will assess nail color, thickness, texture, and growth patterns while reviewing your medical history and possible contributing factors. Because several conditions can resemble a fungal nail infection, a small nail clipping or sample of debris beneath the nail may be collected for laboratory testing. Tests such as fungal staining, culture, or molecular testing can help confirm the presence of fungus and, in certain cases, identify the organism involved.

​Treatment for Toenail Fungus

Treatment for toenail fungus depends on the severity of the infection, the number of nails involved, nail thickness, and your medical history. Options may include:

Topical Antifungal Medications: Prescription solutions or lacquers applied directly to the nail may be appropriate for mild or limited fungal infections.

Oral Antifungal Medications: Prescription medications, such as terbinafine, treat the infection through the bloodstream and are generally more effective for extensive fungal nail infections. Medical evaluation and appropriate laboratory testing may be needed before treatment.

Professional Nail Debridement: Reducing the thickness of affected nails can relieve pressure, improve comfort, and support other antifungal treatments.

Laser Therapy: Laser devices may be considered in selected cases, although this therapy is combined with topical antifungal medication.

Nail Removal: In severe or persistent cases, temporary or permanent nail removal may be considered when other treatments are unsuitable or unsuccessful.

Because toenails grow slowly, visible improvement may take several months, even after the fungal infection has been successfully treated. Complete replacement of an affected big toenail may take 12–18 months.
